Vial labels give milligrams; the amount per draw is usually in micrograms. Converting the vial mass to micrograms first keeps the division honest: 5 mg is 5,000 mcg, so a 250 mcg draw is one twentieth of the vial, and the vial holds 20 of them.

When the vial and the amount are both in the same unit, the number of draws is a plain division and the concentration can be written in mcg/mL directly. That is why the main calculator converts everything to micrograms behind the scenes before it divides.
